In one embodiment, the guide member 816 includes a passage through the guide member 816 defining the cross fixation insertion axis 420 and one or more sleeves 808a,b received in the passage in axial sliding relationship. As used herein, "passage" refers to a duct, a vessel, an opening, a void, or other channel in a body of a an apparatus, instrument, structure, member, device, component, system, or assembly.
