In the illustrated embodiment, the proximal resection feature 1112 guides a cutting tool to form a first osteotomy in at least a medial cuneiform 202. The proximal resection feature 1112 defines a first trajectory for a first osteotomy which is determined based on a bone model of a plurality of midfoot bones. The bone model 404 may be based on medical imaging of the patient’s foot. In one embodiment, the bone model 404 is configured to significantly resemble the anatomy of the patient's foot. Similarly, the distal resection feature 1114 is configured to guide a cutting tool to form a second osteotomy in one or both of a medial cuneiform 202 and a metatarsus (e.g., first metatarsal 208). The distal resection feature 1114 defines a second trajectory for a second osteotomy. The second trajectory is determined based on the bone model 404.
